Dans l'Italie du debut du siècle, agitée par de violents troubles sociaux, Mori, médecin dans un hopital psychiatrique, et sa femme Vittoria tentent de mener une vie sereine. Il y parviennent jusqu'à la naissance de leur enfant. Vittoria n'éprouve aucun sentiment pour ce nouveau-né et refuse de l'allaiter. Elle s'occupe de lui mais ne lui apporte aucune affection. Mori décide alors de faire appel à une nourrice. Il choisit Annetta, jeune et jolie paysanne qui sait etablir un contact affectif avec l'enfant, provoquant ainsi un malaise profond en Vittoria.
