Terry McCaleb, un profiler légendaire du FBI, traque un tueur en série, retors et obstiné, qui s'amuse à le provoquer en semant sur les lieux de ses crimes des messages codés en lettres de sang. Alors qu'il est sur le point de capturer l'insaisissable Code Killer, le policier s'écroule, victime d'un infarctus. Sauvé de justesse par une greffe du coeur, il entame une longue convalescence, et deux ans après son accident, mène dans le port de San Diego, une paisible vie de retraité.
C'est alors qu'une jeune femme, Graciela Rivers, vient solliciter son aide pour lui demander de retrouver le meurtrier de sa soeur, Gloria, celle à qui appartenait le coeur dont Terry est aujourd'hui pourvu. Pour honorer sa "créance de sang", ce dernier se lance alors dans une enquête ardue, physiquement dangereuse, avec le concours de son ami nonchalant Buddy Noone.

It shouldn’t sound like a rubbery balloon being rubbed against your skin. It should sound like flesh ripping or anything more masculine than a clown twisting balloon animals together for you."
