Matt Benson, agent du FBI, a été envoyé en mission sur l'île de Panaréa, dans l'archipel de Lipari, dans le cadre d'un accord entre les gouvernements italien et américain. Une superbe maison face au volcan de Ginostra, ainsi qu'une protection rapprochée, lui ont été allouées, ce qui enchante sa femme Jessie et Tina, leur petite fille de cinq ans.
Quelques jours après leur arrivée commencent les interrogatoires du jeune Ettore Greco, dont la famille a été assassinée par la Mafia. Pour des raisons de sécurité, Matt décide de l'héberger. Cette "adoption" va lentement révéler le terrible secret qui unit l'homme et l'enfant, un secret dont son épouse ne sait rien et qui va conduire toute la famille aux portes de l'enfer. Autour de la maison, le danger se rapproche inexorablement ; à l'intérieur, la peur s'installe peu à peu...
