Le docteur Peralta est assassiné. Les soupçons du policier Stevenson se portent sur Terry Collins, qui aurait passé la soirée avec lui. Mais la jeune femme, choquée quand elle apprend la mort de son ami, a un solide alibi. Stevenson découvre que Terry a une sœur jumelle, Ruth avec qui elle échange souvent son identité et ses activités. Dans ces conditions, une des deux a pu tuer Peralta, d’autant qu’elles sont peu coopératives et protègent leur double intimité. Pour y voir clair, Stevenson demande l’aide d’un confrère de Peralta, Elliott, psychanalyste expert en gémellité, qui connaît et apprécie lui aussi Terry (ou est-ce Ruth ?). Elliott se livre donc, avec leur accord, à des expériences d’apparence scientifique sur elles en fait avec l’idée de résoudre la double énigme que posent les jeunes femmes. Il découvre que, si Ruth est « normale », Terry est psychiquement perturbée et simulatrice, qu’elle s’est toujours sentie repoussée quand sa sœur attirait la sympathie ou l’amour, ce qui justifie les soupçons de Stevenson. Il est urgent de faire éclater la vérité car, si l’une a tué en le cachant à l’autre, l’innocente risque gros aujourd’hui. C’est ce qui se produit : Terry, jalouse de la relation qui se noue entre Ruth et Elliott, torture mentalement sa sœur pour la mener à la folie. Stevenson fait croire à Terry que Ruth s’est suicidée. Terry se fait alors passer pour Ruth et accuse sa sœur. Mais la véritable Ruth réapparaît, bien vivante, et la véritable Terry est confondue. Elliott saura désormais qui est qui…
