1761, Londres. A l'asile Bedlam, le sadique directeur George Sims règne d'une main de fer. Un prisonnier qui essayait de s'évader meurt en tombant du toit du bâtiment. Par hasard, le très riche et puissant Lord Mortimer assiste au tragique événement, en compagnie de Nell Bowen, sa jeune protégée. Le défunt était en réalité un célèbre poète que Mortimer admirait. Bientôt, Nell et le Lord découvrent l'horrible traitement infligé à ces malades mentaux. Bien vite apaisé par les discours de Sims, Mortimer ne semble plus s'intéresser à l'affaire. Mais Nell n'en démord pas et finit par être elle-même internée...
